2. Medical Services Disclaimer

PeakLAB Medicine provides integrative and functional medicine services through licensed medical professionals. The content on this website is for informational purposes only and does not constitute medical advice, diagnosis, or treatment.

No Doctor-Patient Relationship: Browsing this website does not establish a doctor-patient relationship. A doctor-patient relationship is only established upon completion of an intake process, execution of a patient agreement, and acceptance by Dr. Bailey.

Emergency Situations: Our telemedicine services are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.

3. Telemedicine Services

PeakLAB Medicine offers telemedicine services to patients located in Colorado, Arizona, and Nebraska. By using our telemedicine services, you acknowledge and agree that:

• Telemedicine involves the delivery of healthcare services using electronic communications and may not be the same as an in-person visit.
• Technical difficulties may interrupt or prevent completion of a telemedicine visit.
• You are responsible for ensuring you have appropriate technology and internet connectivity for telemedicine visits.
• Some conditions may require an in-person evaluation that telemedicine cannot provide.
• You must be physically located in a state where Dr. Bailey is licensed at the time of your appointment.

4. Membership and Payment Terms

PeakLAB Medicine operates on a membership and fee-for-service model. By enrolling in a membership or scheduling services, you agree to the following:

• Membership Fees: Monthly membership fees are billed in advance and are non-refundable except as required by law.
• Cancellation: You may cancel your membership with 30 days written notice. Cancellation requests must be submitted to [email protected].
• Appointment Cancellations: Appointments cancelled with less than 24 hours notice may be subject to a cancellation fee.
• Insurance: PeakLAB Medicine does not accept insurance. Patients are responsible for all fees at the time of service. We can provide documentation for potential HSA/FSA reimbursement.
• Lab Fees: Laboratory tests ordered through PeakLAB Medicine may be billed separately by the laboratory and may or may not be covered by your insurance.

5. Intellectual Property

The content on this website, including text, graphics, logos, images, and software, is the property of PeakLAB Medicine and is protected by applicable intellectual property laws. You may not reproduce, distribute, modify, or create derivative works without our express written permission.

6. Limitation of Liability
