It does not govern information collected through our electronic health record and patient management platform, Practice Better, which is subject to a separate Business Associate Agreement and our HIPAA Notice of Privacy Practices.   • Contact Form: Name, email address, phone number, state of residence, and the content of your message.
  • AI Chat Assistant: The text of your chat conversations with our virtual assistant. These conversations are used to respond to your inquiries and may be reviewed by our team to improve the service. The chat assistant does not collect or store protected health information.
  • Discovery Call Booking: When you book a Discovery Call or appointment via Practice Better, you provide your name, email, phone number, and appointment preferences. This information is processed and stored by Practice Better, not directly by this Website.
  • Email Communications: If you email us directly, we retain the content of your messages and your contact details.

Information Collected Automatically

When you visit the Website, certain information is collected automatically through cookies and similar technologies:

  • Usage Data: Pages visited, time spent on pages, referring website, browser type, operating system, device type, and IP address.

Important: This Website does not collect, store, or transmit Protected Health Information (PHI) as defined by HIPAA. No medical records, diagnoses, lab results, treatment plans, or clinical communications are processed through this Website.

PeakLAB Medicine is a HIPAA-covered entity. All PHI related to your care — including intake forms, clinical notes, lab results, and secure messages — is handled exclusively through Practice Better, our HIPAA-compliant electronic health record and patient management platform. Practice Better operates under a Business Associate Agreement with PeakLAB Medicine.

The AI chat assistant on this Website is designed to answer questions about the practice, services, and scheduling only. It is explicitly prohibited from collecting, discussing, or storing any health information. If you share health information in the chat, it will not be used for clinical purposes and you will be directed to contact Dr. Bailey's team directly.
