After booking, you'll receive a comprehensive intake form via email. Complete it before your first appointment so Dr. Bailey arrives fully prepared.
If appropriate after your consultation, Dr. Bailey may recommend laboratory testing or other next steps tailored to your history, goals, and care plan.
Your Initial Consultation is a 90-minute Zoom visit with Dr. Bailey to review your history, questions, and appropriate next steps.
When follow-up care is appropriate, Dr. Bailey will explain the recommended cadence and how to use the secure Patient Portal.
Care conversations happen remotely, without a commute to Fort Collins or a waiting room.
