Annual-plan amounts are monthly equivalents for comparison; the annual plan is paid in full once each year. Monthly plans are billed each month. Every membership requires a one-year commitment from the join date. Annual payment saves about 10% compared with 12 monthly payments.

## Important pricing boundaries

- Membership pricing does not include medications, labs, or supplements. Those items are billed separately by the pharmacy or laboratory, with the applicable membership discount applied.
- Unused visits do not roll over.
- Pinnacle same-day scheduling is subject to availability.
- PeakLAB does not bill insurance. HSA/FSA card use and any reimbursement depend on the client's plan.
- Final cancellation and refund terms are provided before enrollment or booking.
- Hormone and weight-management treatment, testing, prescriptions, and follow-up frequency are individualized. They are not guaranteed by a membership tier.
